Admission Test Procedure for Teacher’s Certification Program

1. Register Online
   
   • Online register at https://myschool.carsu.edu.ph/index.php... by filling out the application for admission form.
   • Click save and take note of the Application Number.
2. Set an Appointment
   
   • Set an appointment at https://admission.carsu.edu.ph/sites/online_appointment/#
   • Click SIGN IN.
   • Use your application number as your username and your date of birth (YYYY-MM-DD) as password to Sign In at Online Appointment page.

(Example: Username: 2021-00001 | Password: 2009-01-29)

3. Secure a Referral Form
   
   • Visit the College of Education and secure a Referral Form.
4. Pay test fee
   
   • Pay test fee at the University Cashier amounting 400.00 pesos.
   • Present your application number upon payment.
5. Show up on your Appointment date (Examination Date)
   
   • Show up on your appointment date and bring the following:

Official Receipt, Referral Form, 1pc. Passport Size ID Picture (No Picture, NO EXAM), Valid ID (ex. School ID, Voter's ID, Voter's Certification, Driver's License, Postal ID, Passport or any Government issued; NO ID, NO EXAM), Pencil (number 2, any brand), Ballpen, Face Shield, Face mask, alcohol/hand sanitizer.
