Requirements for Exam:

1. Duly filled-out Application Form
2. 2 copies of recent 1x1 ID picture
3. P250.00 (Undergraduate Programs) Examination Fee (Pay at the cashier)
   P400.00 (Graduate Programs) Examination Fee (Pay at the cashier)
4. 1 Valid Identification Card with picture
5. 2 No. 2 Pencil with eraser
