- Photocopy of High School Report Card OR Certification from the High School Principal that the student is a graduating student
- Transfer of Credentials (For Transferring Students only)
- Two (2) copies of 1X1 ID picture attached to the marked places of the Application Form
- Testing Fee (Non-Refundable)
- P250.00 (for undergraduate programs)
- P400.00 (for graduate programs)
- Duly Accomplished Form
