Downloadable Forms

Use this page to download forms offered by the Human Resources Management Office (HRMO). You can print and reproduce these forms but you are prohibited to sell it.

Step 1. Make sure you have the following softwares:

If your system is running a Linux Operating System (i.e., Bayanihan Linux, Fedora Core, Ubuntu, etc.) you don't have to download and install any software package to open these files because these softwares are usually pre-installed. You can also open these files even though you are running a Linux Operating system in your desktop.

If your system is running a Windows Operating System (i.e., Windows 98, ME, XP or Vista), you will need the following softwares:
- Adobe Acrobat Reader, to view and print files of type .pdf (Portable Document Format).
- Microsoft Office Suite (Microsoft Word, Excel, Powerpoint, etc.), to open, edit, and print files of type .doc, .xls, and .ppt, respectively.

Step 2. Download, save then open the files below: (Click on the name of the file you wish to download.)
