Conditions and Reminders

Pursuant to the existing policy of the University on Confidentiality of Student Records, only the owner of the record is allowed to request its release and claim the same.
If for any reason the owner of the record cannot personally request the release or claim it, they may authorize a person of legal age to do so on their behalf. The owner must execute a written authorization, affixing their original signature. This authorization must be accompanied by an original and photocopy of valid identification (ID) for both the record owner and the authorized person. The original IDs are required for comparison purposes and must be presented when the authorized person claims the records.
For claiming government CAV through an authorized person, the authorization must be duly notarized by a legitimate lawyer. Additionally, original and photocopy IDs of both the authorizing individual and the authorized person must be presented upon the release of the requested record(s).
Documents not claimed within sixty (60) days will be destroyed.
The release of Certificate of Transfer of Credentials/Honorable Dismissal shall only be done ONCE. Caraga State University will NOT issue any other academic records without the issuance of a certificate/letter of No Objection from the current admitting school or a Certificate of Transfer Credential, if applicable.
The release of requested records is scheduled from 1:00 PM to 5:00 PM. Failure to claim the records as scheduled will result in their release from 1:00 PM to 5:00 PM on the next succeeding working day.
For clients who choose to pay the corresponding fees for the release of academic records at any CSU accredited online payment center, please note that the date of request filing will be considered the day you email payment.csuour@gmail.com a copy of the Official Receipt (OR) for the transaction. If you email it on a non-working day, it will be considered filed on the following working day.
Contact information provided in this request slip will be collected and shared with other regulatory government agencies for the purpose of evaluating the services rendered by the office.
