Steps in Filing and Claiming of Requested Academic Records
The list below is the type of Academic Records, but not limited to:
- Transcript of Records (TOR)
- Transfer Credential (Honorable Dismissal)
- Authentication
- Diploma
- Reconstruction of Diploma
- Certification, Authentication and Verification (CAV)
- Certification related to Academic Records
- Certification of Registration (COR)
- Online Grades
The following steps and procedures are extracted from the University’s Citizen’s Charter:
Step 1
Access, register, sign in, and book to the appointment system.
Step 2
Arrive at CSU in the scheduled appointment and approach the In-charge at Window 1 in the Office of the University Registrar.
Step 3
File for the request of the release of academic records.
Step 4
Pay the assessed fees in the cashier’s office.

Step 5
Submit the following to the Receiving Window In-charge of the Office of the Registrar:
- Official Receipt of Payment for the assessed fees
- All the documentary requirements
Step 6
Wait and pay attention to the schedule reflected in the issued “Claim Stub“.
Step 7
Arrive at CSU on the specified schedule in the Claim Stub and approach the Releasing Window In-charge at the Registrar’s office to claim the requested academic record(s). In doing so, all the required documents must be presented to the said in-charge.
